1. EIN and LLC name mismatch

If the IRS-registered name doesn't exactly match the Walmart form, the system auto-flags. Copy the spelling letter-for-letter from your CP575 or 147C letter.

2. Personal banking instead of business

Applying with a personal bank account is a rejection trigger. Real business accounts (Mercury, Relay, Wise Business) are required; the account address should match the EIN.

3. W-9 inconsistency

Wrong entity type marked on the W-9 (single-member LLC vs. C-Corp vs. partnership) stalls the process. It must match the entity type the EIN was issued for.

4. Suspicious website

Brand-new domains, thin content, or generic templates lower the trust score. A site with at least 30 days of age and real product/business information is recommended.

5. IP / device fingerprint

Leftover traces of a previously rejected account (cookies, IP, browser fingerprint) hurt new applications. A clean device + clean IP is mandatory.

6. Missing category approval

Required category permission not selected leaves the account half-set. Restricted categories may require extra documentation.

7. Address verification

Unclear utility bill or bank statement, or one older than 90 days, holds approval. PDF quality matters too.
